Information you may provide
The Parent Voice form asks for a general topic or suggestion. Your name and email address are optional. Please do not include a child's name, student ID, diagnosis, classroom, teacher or staff member's name, or other confidential or sensitive information.
How submissions are handled
Form submissions are processed through Netlify Forms and can be viewed by the website administrator in the site's Netlify dashboard. If email notifications are enabled, submission details may also be sent to the notification email account. Anyone with access to the Netlify site or that email account may be able to view the submission.
Do not assume a submission is anonymous simply because your name and email are optional. Avoid including details that could identify a student or family.
How information is used
Submissions are intended to help the parent advocacy group identify general topics, questions, and resource ideas. Do not submit emergencies, urgent safety reports, complaints requiring formal school action, or confidential student records through this form. Contact the school or appropriate emergency service directly when needed.
Retention and deletion
The site administrator should review submissions regularly and delete them when they are no longer needed, subject to any applicable recordkeeping obligations. Copies may remain in email inboxes or other systems where notifications were delivered. To request deletion or ask a privacy question, contact the site administrator using the contact method published on the main website.
Security and sharing
Access to submissions should be limited to people who need it for the group's work. Use a strong, unique password and multi-factor authentication where available for Netlify and the email account. Do not publish raw submissions or share identifiable family information in meeting notes, social media, or public updates.
No website or email system can guarantee absolute security. Submit only the minimum information needed.
Changes to this notice
This notice may be updated as the website's practices change. The site administrator should keep the notice accurate and add an appropriate contact email or contact method before public launch.
